Having worked with people referred by their physicians, healthcare providers and others who have been helped from all over the country, I have been motivated to create convenient and effective methods of working with individuals living far away.  SKYPE (www.skype.com) is free, completely free, and easy to use.  I am very technologically-challenged and I use it.  As well, there is always the phone.
