If you are using www.mapquest.com or a GPS device, please input 2 Ohio Drive, New
Hyde Park, NY as the actual street address. 2 ProHEALTH Plaza is the address on the
awning of the building. If you follow the directions below, look for the 2 ProHEALTH Plaza awning.
PLEASE NOTE: New Office Location in 2 ProHealth Plaza:
Due to the addition of several physicians to the 2 ProHEALTH Plaza building, my office has been relocated. I remain
on the 2nd floor. Enter the ProHEALTH suite through the glass doors and immediately on your right there
are blue seats attached to the wall. There is no need to sign in. Please take a seat and I will greet you
at the scheduled time of your appointment.
Due to a lack of parking spaces, please leave an additional 15
minutes to find a place to park.
FROM THE WEST:
LONG ISLAND EXPRESSWAY: Take the Long Island Expressway heading east to Exit 33, Lakeville Road. Make
a right onto Lakeville Road. Continue South, approximately 1 mile, going over the Northern State Parkway. The next
main street after the Northern State Parkway is Marcus Avenue. Make a left onto Marcus Avenue. Take Marcus Avenue
down approximately 4 traffic lights until you see the entrance to 3000 Marcus Avenue on your left hand side. Make a left
into 3000 Marcus Avenue. Continue straight ahead to the security booth. At the security booth, make a right. You
will pass Parker Jewish Rehabilitation Institute on your left, and a lake on your right. 2 ProHEALTH Plaza is the first
building on your right hand side. We have a blue and white awning.
NORTHERN STATE PARKWAY: Take the Northern
State Parkway heading east to Exit 25, Lakeville Road South. The first light after the exit
is Marcus Avenue. Make a left onto Marcus Avenue. Take Marcus Avenue down approximately 4 traffic lights until you
see the entrance to 3000 Marcus Avenue on your left hand side. Make a left into 3000 Marcus Avenue. Continue straight
ahead to the security booth. At the security booth, make a right. You will pass Parker Jewish Rehabilitation Institute
on your left, and a lake on your right. 2 ProHEALTH Plaza is the first building on your right hand side. We have
a blue and white awning.
FROM THE EAST:
LONG
ISLAND EXPRESSWAY: Take the Long Island Expressway heading
west to Exit 34, New Hyde Park Road. Make a left turn at the traffic light onto New Hyde Park Road. Go
over the Northern State Parkway. Continue to the second traffic light after the Northern State Parkway. At the second
traffic light, you will make a right turn into the Lake Success Quadrangle Parking Lot. Make an immediate left and continue
straight ahead around the bend. 2 ProHEALTH Plaza is the last building on your right hand side. We have a blue and
white awning.
NORTHERN STATE PARKWAY: Take the Northern State Parkway heading west to Exit 26 South, New Hyde Park
Road South. Take New Hyde Park Road to the second traffic light and make a right turn at the second traffic
light. You will be turning into the Lake Success Quadrangle. Make an immediate left once in the quadrangle and proceed
straight ahead. Follow the bend all the way down until you cannot go anymore. 2 ProHEALTH Plaza is the last building
on your right hand side. We have a blue and white awning.
